TRYOUT REQUIREMENTS
In order to participate in any sport at Palm Beach Central, please complete the following items:

1) Athletic Eligibility Packet 2021-2022 (updated 8/31/21) - must submit original on paper including notary stamps on pages 3, 5, and 7

2) Covid Release Form - 2021-2022 form within above packet

3) NFHS Courses (Sudden Cardiac Arrest, Heat Illness Prevention, and Concussion for Students - must be completed after April 1, 2021 for the 2021-2022 season) - submit on Team Google Classroom

4) Insurance payable at School Cash Online (opening early July 2021)
